St. John’s Academy Forms

The following forms are used at St. John’s Academy. You have the option of printing them off and sending or bringing them to the school office, or returning families may fill out the St. John’s Academy Registration form online. We ask that if you are a new family to St. John’s Academy, you make an appointment to meet with the principal before registration will be completed.
